Our main objectives are:
1. Lessen the complexity of health care for patients recovering from acute illnesses but with persistently debilitating symptoms
2. Prevent hospitalizations, decrease patient and government health insurance costs, and
3. Correctly identify these patients' needs for mental health care. In order to complete the evaluation of these patients with persistent complaints, the tool will also be able to gather and store a database with multidisciplinary professionals (physiotherapists, nurses, nutritionists, doctors, etc.).
